Registration & Salary Packaging

UWA Staff registering for Corporate Fitness are required to be members of the UWA Sport & Recreation Association ($50 annually). If not already a member, you may include this in your registration.

To register for UWA Staff Corporate Fitness: 

1. Choose your membership or package from the Program Options
2. Complete the Registration Process:
   • Unlimited UWA Staff Corporate Fitness Membership or Package Registration for Personal Training and/or Group Fitness Classes
   • If registering for one of the two 'memberships' available you may salary package your payments. (Please note: 10 week packages cannot be salary packaged). To arrange salary packaging please complete the following:
   • UWA Recreation & Fitness Centre Membership Registration
   • Salary Packaging Agreement (required for staff who do not have a salary packaging agreement in place)Complete the Physical Readiness Questionnaire
   • UWA Staff Corporate Fitness Salary Packaging Form
3. Submitting your forms:
   • Fax your registration forms and, if applicable, your salary packaging forms to 6488 4725 or
   • Scan & email your forms to admin@sport.uwa.edu.au or
   • Visit the UWA Watersports & Leadership Complex (Hackett Drive, Crawley) or
   • Send via internal mail to UWA Staff Corporate Fitness, UWA Sport & Recreation Association, Attention: Jay-Lee Longbottom, M412
